Pigmentation after the Dubai summer: how Alma Harmony XL removes sun spots and why melasma needs a different protocol

By September most Dubai residents notice the same thing in the mirror: new brown spots on the cheekbones, a darker "mask" across the forehead, old acne marks that refuse to fade. Five months of UV index 10-11 leave a trace even on skin that was diligently protected. The good news is that October opens the treatment window. The important caveat is that "pigmentation" is not one condition, and the same light that erases a sun spot in two sessions can make melasma worse.

Three kinds of dark spots, three different problems

Before choosing a device, a doctor needs to answer one question: what exactly is this pigment and how deep does it sit? In practice, three conditions cover most cases we see after the summer.

Solar lentigines (sun spots, age spots). Flat, well-defined brown macules from a few millimetres to over a centimetre, on the cheekbones, temples, hands and shoulders. They are a direct record of accumulated UV exposure and do not fade on their own. The pigment is epidermal, which is why lentigines respond best to light-based treatment.

Melasma. Symmetrical, map-like patches on the cheeks, forehead, upper lip. Melasma is driven not only by UV but by hormones (pregnancy, contraceptives), heat and visible light. According to DermNet, it exists in epidermal, dermal and mixed forms, and the mixed form is the most common; dermal pigment looks blue-grey and responds poorly to any treatment. Melasma darkens with every summer and lightens only partly in winter.

Post-inflammatory hyperpigmentation (PIH). Marks that follow the exact shape of a previous lesion: an acne spot, a scratch, a burn, an overly aggressive peel. Common in darker skin types (Fitzpatrick IV-VI), which describes a large share of Dubai residents.

At the consultation we look at the skin under a Wood's lamp and a dermatoscope. Epidermal pigment becomes sharply accentuated under the lamp, dermal pigment barely changes contrast. That observation decides whether you need a short course or a long one, and which module we start with.

Which Harmony XL modules work with pigment

Alma Harmony XL is a modular platform: one base, many handpieces. Two of them are the working tools for pigmentation.

Dye-VL (AFT technology). A narrow band of pulsed light in the 500-600 nm range with contact cooling. Alma calls the approach AFT, Advanced Fluorescence Technology: the applicator converts short, unusable wavelengths into the useful part of the spectrum and delivers the energy as a uniform pulse. The band is absorbed by melanin and haemoglobin, so it clears sun spots, freckles and the red-brown mottling of photodamage in one treatment. According to the manufacturer's clinical partners, lentigines are visibly cleared in one to three sessions; the treated spots darken for 3-7 days, then flake off. This is the module for solar lentigines and general sun damage.

ClearLift (Q-switched Nd:YAG 1064 nm). A non-ablative laser that delivers very short, photoacoustic pulses. At 1064 nm the energy passes through the epidermis and shatters pigment into fragments that the body clears over weeks, without heating the surface. In the research literature this approach is known as low-fluence "laser toning". A 2022 systematic review of low-fluence QS Nd:YAG for melasma found a significant reduction in melasma scores across studies, with protocols of 5-15 weekly or biweekly sessions. Because 1064 nm is poorly absorbed by epidermal melanin, it is the safer choice for darker skin types, where broad-spectrum light carries a higher burn and PIH risk.

Put simply: Dye-VL removes what is on the surface, ClearLift works deeper and on skin that cannot tolerate bright light. For textured post-acne marks we may add the fractional ClearSkin module described in our NoFilters article.

Why melasma needs a different protocol

This is the point most clinic pages skip. Melasma is not a spot to be erased; it is a chronic condition of overactive melanocytes. The same systematic review that confirmed the efficacy of laser toning also reported recurrence rates of 64-81% at three months in several studies when the laser was used alone, and a 5-11% risk of mottled hypopigmentation with aggressive protocols. Intense pulsed light used alone on melasma can trigger rebound darkening.

That is why in SOLAR melasma is treated as a combined programme, not a course of flashes:

  1. Skin preparation, 3-4 weeks. Topical depigmenting agents prescribed by the doctor (tranexamic acid, azelaic acid, retinoids or, where appropriate, hydroquinone-based combinations) quiet the melanocytes before the first laser session. A 2025 meta-analysis of 11 randomised trials found that laser or light combined with topical agents outperformed laser alone, with the gap widening from week 8 to week 16.
  2. Low-fluence ClearLift. Gentle passes at 1-2 week intervals, usually 6-10 sessions, without trying to "bleach" the patch in one visit. Dye-VL is used on melasma only in selected cases, on the lowest settings, and never in a season when the client will be in the sun.
  3. Maintenance. After clearance, topicals continue in a lighter regimen, with a maintenance laser visit every 2-3 months during the autumn and winter season.
  4. Photoprotection that includes visible light. Standard SPF blocks UV but not the blue-violet light that also stimulates melanocytes. A double-blind randomised trial in 68 melasma patients showed that a sunscreen containing iron oxide (visible-light protection) improved MASI scores by 77.8% versus 61.9% with UV-only sunscreen over 8 weeks. A tinted, iron-oxide SPF 50+ is a treatment component, not a cosmetic choice.

Sun spots, by contrast, are a short project: two or three Dye-VL sessions and they are gone.

Why autumn is the right time to start

Dubai's UV index tells the story. Typical midday peaks by month: January 5, February 6, March 8, April 9, May 10, June 11, July 11, August 10, September 9, October 7, November 5, December 4. Light-based treatments are not performed on tanned skin, because the device cannot tell the melanin in a spot from the melanin in a tan; the standard requirement is 2-4 weeks without active tanning before a session and strict sun avoidance for about two weeks after. From October to March that is easy to keep. Starting in June means fighting the sun during every interval between sessions.

A pigment course started in October is typically completed by January, leaving the skin two protected months before the UV index climbs again in March.

Preparation and aftercare

Contraindications include pregnancy, active tan, photosensitising drugs, recent isotretinoin, active skin infection and a history of keloids. Cost depends on the area and number of sessions and is confirmed at the consultation.

FAQ

Will one session remove my sun spots?

Often one Dye-VL session clears most lentigines, but the manufacturer's data and our practice suggest planning for 1-3 sessions 3-4 weeks apart. Deeper or older spots need the additional visits.

Can melasma be cured?

No, and any promise otherwise should make you cautious. Melasma can be brought to a state where it is barely visible and kept there with topicals, maintenance sessions and visible-light photoprotection. Recurrence after laser alone is the rule, not the exception.

Is laser safe for darker skin?

The 1064 nm ClearLift is the standard choice for Fitzpatrick IV-VI because it bypasses epidermal melanin. Dye-VL is used on darker skin with conservative settings and a test spot. The consultation determines which is appropriate.

How soon after the beach can I be treated?

At least 2 weeks without new tan, preferably 4. Come to the consultation in any case: we can start topical preparation while the tan fades.

Can I combine this with HydraFacial or Virtue RF?

Yes, with spacing. HydraFacial is a good preparation step a week before light treatments. Virtue RF microneedling is scheduled in a separate block so the skin is not stressed twice in one week.
